Upon submitting your name, email address to be used for communication, phone number and accepting the agreement below, you will be provided with the physician's email address. EXISTING PATIENT/PARENT - PHYSICIAN ELECTRONIC COMMUNICATION AGREEMENT If you would like to schedule an appointment, please call the office (do not use email). Email must not be used for urgent matters; email messages should be concise. Questions and updates may be submitted. Email should not be used for subject matter that would normally require a lengthy discussion; its intended use is for direct questions. Email sent by new patients never seen in the office will not be returned. Privacy and confidentiality issues: email exchanged over public servers, although generally deemed privacy sufficient, is not without its risks which include, but are not limited to illicit interception of private data. Other potential issues include incorrectly addressing an email – thus resulting in delivery to an unintended recipient. Risks also include technical interference resulting in delayed or undelivered email. Pediatric and Adult Asthma, Allergy & Immunology, LLC will be held harmless for information loss due to technical failures. Email has other inherent limitations such as validating the identity of the communicating parties. If appropriate, email addressed to Dr. Lisa Barisciano may also at times be viewed/responded to by office staff. If office staff responds to an email, they will identify themselves in that email. All computers/devices/accounts permitting access to patient-physician electronic communications are password-protected. Make appropriate use of the email subject line when writing the physician, thus permitting the triage of messages. Ensure the patient name and information relevant to the question or update is included in the email. If an update is provided and a question is not asked/a response is not requested, a response will not be generated. Patient-identifiable information is never forwarded to a third party without the patient’s express permission. Marketing materials from our office will never be sent and email accounts will never be shared. All “To” fields are double-checked prior to sending messages. By selecting the “I accept” button, you acknowledge accepting these limitations prior to communicating privileged information via email and that you will abide by the general guidelines summarized here.
